The Canadian Mental Health Association provides a variety of resources and community outreach for individuals that require support in securing and maintaining safe and stable housing.
Housing Outreach Services
The Homelessness Prevention Team, a partnership with Social Services – Sault Ste. Marie District, provides a Housing First approach with wrap around intensive housing based case management to those who are experiencing homelessness and those who are at risk of homelessness.
Staff work within a multi-agency framework that provides the following services and supports:
- Street, community and housing outreach
- Case management
- Landlord mediation and assistance
- Discharge planning from various institutions
- Information and referrals
- Family support services
- Coordination and interaction with other services and organizations
- Mental health and addiction supports to the shelter system
The Canadian Mental Health in a joint partnership with Sault Area Hospital, Algoma Public Health, Ontario Aboriginal Housing Services as well as Social Services of Sault Ste. Marie (Housing Support) provides daily resources and staff members to assist each individual residing at 137 East St.
Our Willow Haven program is a supportive housing facility located at 345 St. George’s Avenue in Sault Ste. Marie, Ontario. The program currently houses 10 residents and is staffed 24 hours per day 7 days per week with Housing Support Workers.
Our priority is to assist in resident’s personal recovery by providing support in many ways, including: assistance in medical and mental health issues, facilitating appointments, daily living skills, addictions support, family relationships, vocational skills, education and more.
We are continually working on establishing connections within the community to ensure our residents receive support in all areas where they have identified a need. We are currently working with many community services, liaising with other health care professionals and service providers (doctors, psychiatrists, vocational, educational, addictions.)
We provide programming throughout each day, including weekends that involve various aspects of personal health and well-being, including: interpersonal skills, maintaining hygiene, maintaining wellness, self -esteem, mindfulness, and smoking cessation. We also provide social recreation through nature walks, art, music nights, game nights, and various community outings.
We provide a varied and nutritious menu of home cooked meals that the residents participate in cooking. This allows for a home-like feel to our facility and some hands-on life skills training. We also have full, on-site laundry facilities and provide assistance, when needed, with completing laundry chores in a timely and efficient manner. The program also provides assistance with housekeeping within the residents units.
